2008 YLIF - Youth Legal/Insurance Form - Required for members of Village delegations, Summer Camp delegations, Interchange delegations and International Youth Meeting delegations

2008 TWAL - Traveling without Adult Leadership form - Required for Junior Counselors, Seminar Camp participants and International Youth Meeting participants who are age 20 or younger and are not part of a delegation

2008 ALIF - Required for all participants age 21 or older

Health Form - Physician's form required for all participants in all programs
